Carolyn was born on July 24, 1947 to Everett and Flora (Thompson) Collins.
She was preceded in death by her parents; husband, Richard Moffett; 4 brothers, Randy, Robert and infants, Keith and Kelly; 2 sisters, Ann Turnbull and Mildred Harmon.
Carolyn was a member of the Turney United Methodist Church.
Survivors: 2 sons, Pete (Deborah) Moffett, Portland, OR, Zachary (Jennifer) Moffett, Kearney, MO; 5 grandchildren, Cayden, Tristan, Elizabeth, Catherine and Victoria; 1 great-grandchild, Harley; step-daughter, Terry Matthiesen, Johnsburg, IL; 7 brothers, Orville “Dean” (Schelia)Collins, Tommy (Robin) Collins, Mikel (Chris) Collins, Ronald Collins, Ranol (Michelle) Collins, Steavy (Cathy) Collins, and Bruce Collins; 3 sisters, Nancy (Fred) Sympson, Thelma (Quinn) Caldwell and Belinda (Bob) Kuntz; sisters-in-law, Betty and Margie Collins and brothers-in-law, Ralph Turnbull and Mark Harmon.
Services: 7:30 PM, Thursday, September 25, 2014 at Poland-Thompson Funeral Home, Cameron. Visitation: 6:30-7:30 PM, prior to the service.
